
My Story
Kathy Zeleskey has served in the Hospice Industry for the majority of her professional career. She loves what she does! It is very rewarding for her to have the opportunity to use her knowledge and experience of almost 30 years to help patients and families access the care they need WHEN they need it the most. In addition to WHERE they need it the most, which is most commonly in their own homes.
Because there are so many misconceptions surrounding Hospice, education in the community is an integral part of what Kathy does on a daily basis - education with community partners, referral sources, patients, and families.
Kathy has been very successful partnering with Physicians' offices, Hospital Social Workers and Case Managers, Long Term Care, Assisted Living and Independent Living Facilities, Personal Care Homes, and many other referral sources within the community. Her referral sources call upon her when their patients are no longer receiving aggressive/curative treatment. They view her as a resource for arranging specialized patient care as an extension of their practice or services.
